Assessment & Eligibility

“It is clear that Liberty has an outstanding and highly professional team in place…The much improved atmosphere and relationship with providers has been both cooperative and appropriately independent…This, taken together with the talents of the team, has led to consistently clear, candid, and useful dialogue, and effective problem solving.”

– Assistant Director, Medicaid Community Services

Liberty can help states to control Medicaid costs and assure appropriate levels of care with a rigorous independent assessment program for a wide range of beneficiaries who may be eligible for Medicaid services. In one of our programs for assessment of home-based personal care services, nearly 100 mobile Liberty clinicians assess over 50,000 individuals annually, who, because of a physical disability, cognitive impairment, or chronic medical condition, are unable to accomplish the Activities of Daily Living (ADLs) of bathing, dressing, mobility, toileting, and eating. We deploy nurses on a statewide basis to assess individuals in private residences, licensed adult care homes, family care homes, and supervised living homes, to determine if they qualify for needed personal care services. This entails the capability of operating a large customer call center for receiving referrals and processing questions from clients and providers.
